Elder Empowerment Services offers private duty nursing at home for adults and older adults who need dedicated one-on-one nursing support, along with private-pay RN care management and selected skilled nursing visits throughout Metro Detroit. Our nurses can help families manage complex needs, follow an established medical plan, monitor changes, communicate with authorized providers, and provide extended nursing coverage in the client’s residence.

When an older adult’s needs become more complex, families often need more than a caregiver visit. They need a licensed nurse who can look at the full picture, identify concerns, organize the care plan, communicate clearly with the medical team, and help the family decide what to do next.

Our approach is practical, compassionate, and family-centered. We can help with nursing assessments, ongoing oversight visits, care coordination, medication review, placement navigation, and selected hands-on treatments. Every client is reviewed individually. Clinical services are scheduled only when the service is appropriate, the assigned nurse is qualified, required orders are in place, and the case is accepted by our clinical team.

Private Duty Nursing at Home

Private duty nursing provides dedicated nursing care for an approved block of time in the client’s home. It is appropriate for clients whose needs require more clinical attention than ordinary personal-care or companion services can provide. Care is delivered by a Michigan-licensed nurse according to the individualized care plan, current provider orders when required, nurse competency, staffing availability, and clinical acceptance.

Private duty nursing may include:

• One-on-one nursing observation and ongoing assessment during the scheduled shift.
• Medication administration and injections according to current orders.
• Diabetes monitoring and ordered insulin support.
• Ordered wound, catheter, ostomy, post-operative, or chronic-condition care.
• Monitoring for changes in condition and communicating authorized concerns to the physician or other provider.
• Caregiver teaching, discharge-plan reinforcement, documentation, and coordination with the family.
• Selected infusion or vascular-access support only after a separate clinical, pharmacy, order, competency, and insurance review.

Simple private duty nursing pricing:

• $150.00 per hour
• No minimum-hour requirement

Short visits and longer shifts can be scheduled according to the client’s care needs. Medications, treatment supplies, travel outside the normal service area, and after-hours premiums may be separate.

Private duty nursing is scheduled care, not emergency response or automatic 24/7 on-call coverage. If a client has a medical emergency, call 911.

What Private RN Care Management Can Include

• Comprehensive RN assessment: review of health history, current concerns, medications, mobility, fall risk, home safety, caregiver capacity, and care priorities.
• Personalized nursing care plan: clear recommendations for what the client needs now, what the family should monitor, and when the plan should be updated.
• Ongoing nurse oversight visits: scheduled follow-up visits to assess changes, review the effectiveness of the plan, educate caregivers, and communicate concerns.
• Physician and provider communication: organized updates, appointment preparation, medication questions, and coordination with authorized providers after the client signs the required release.
• Medication management support: medication reconciliation, education, adherence review, mediset review, refill coordination, side-effect monitoring, and reporting concerns to the prescriber.
• Hospital-to-home support: discharge-instruction review, follow-up coordination, home-safety planning, caregiver teaching, and help identifying gaps that could lead to a preventable setback.
• Family communication: a clear written or verbal update to the authorized decision-maker after the visit.
• Caregiver teaching and supervision: task-specific education and oversight when appropriate and permitted.

Families who also need hands-on personal care, homemaking, respite, or companionship can review our home-care services. RN care management can be purchased as a separate private-pay service or coordinated with an existing Elder Empowerment home-care plan.

Help With Senior Living and Care Placement

Choosing between remaining at home, adult foster care, assisted living, memory care, rehabilitation, or a skilled nursing facility can feel overwhelming. Our placement navigation service helps families organize the decision and compare realistic options based on care needs, budget, location, safety, accessibility, and family preferences.

Placement help may include an RN needs assessment, review of recent records, creation of a short list, questions to ask each community, tour or meeting support, transfer coordination, and a post-move follow-up. We do not guarantee acceptance, bed availability, payer approval, or a particular outcome. If Elder Empowerment or its owner has a business relationship or ownership interest that could create a conflict, that relationship will be disclosed so the family can make an informed choice.

Selected Skilled Nursing Visits

Selected hands-on nursing services may be available after clinical review. A valid written order from an authorized prescriber is required whenever the treatment, medication, dose, route, frequency, or clinical parameters require one. Services are also subject to nurse competency, insurance coverage, staffing, medication and supply availability, and the safety of the home setting.

• Wound assessment and ordered wound care: wound description and measurement, dressing changes, skin-protection teaching, infection-risk monitoring, caregiver education, and provider updates.
• Medication administration and injections: selected prescribed medications or injections, including ordered insulin administration, when the medication, supplies, and written directions are available.
• Diabetes and insulin support: medication reconciliation, glucose-log review, ordered monitoring, insulin teaching, storage and supply review, and hypoglycemia or hyperglycemia escalation planning.
• IV infusion nursing: selected infusions may be considered only after the prescriber’s order, medication source, pharmacy responsibilities, vascular-access plan, emergency protocol, and nurse competency are confirmed.
• Additional skilled visits: selected catheter, ostomy, specimen-collection, post-operative, or chronic-condition support may be considered case by case.

We do not prescribe medications, independently change doses, provide medical diagnoses, guarantee results, or replace emergency services. If a client has chest pain, severe difficulty breathing, signs of stroke, uncontrolled bleeding, a serious medication reaction, or another emergency, call 911.

Starting Private-Pay Rates

The following rates are starting prices for the normal Metro Detroit service area. Final pricing depends on complexity, total professional time, travel, supplies, after-hours needs, and whether another clinician is required. A card on file and signed service agreement may be required before the first visit.

Placement navigation: $325 for the RN placement assessment; $150 per professional hour after the assessment; or $1,250 for a comprehensive package that includes the assessment and up to six additional professional hours. Extra time is $150 per hour.

Other fees: travel within 20 miles of the normal service area is generally included. Additional mileage may be $1 per mile. Same-day, weekend, holiday, or after-hours requests may add 25%. Cancellations with less than 24 hours’ notice may be charged 50%; cancellations with less than four hours’ notice may be charged the full scheduled fee. Rates and availability are subject to change and will be confirmed in writing before service begins.

Monthly RN Oversight Options

Some families prefer a predictable monthly plan instead of scheduling each visit separately. Monthly options are available after the initial assessment:

• Essential Oversight — $395 per month: one 60-minute RN visit, up to 60 minutes of care coordination, and one monthly family summary.
• Active Management — $725 per month: two 60-minute RN visits, up to two hours of care coordination, monthly care-plan review, and family summary.
• Complex Care Management — $1,295 per month: four 60-minute RN visits, up to three hours of care coordination, monthly interdisciplinary review, and family summary.

Unused time does not roll over. Procedures, supplies, urgent responses, travel outside the normal service area, and after-hours work are billed separately.

How the Process Works

1. Call to discuss the need. Tell us what is changing, who is involved, where the client lives, and what help the family is seeking.
2. Clinical screening. We determine whether the request is appropriate for RN care management, ordinary home care, an ordered skilled visit, another provider, or emergency evaluation.
3. Assessment and authorization. The client or legal representative signs the service agreement, privacy and communication authorizations, and other required consent forms.
4. Orders and records. When hands-on treatment is requested, we verify the current medication list, orders, provider contacts, pharmacy responsibilities, and supplies before the visit is confirmed.
5. Visit and documentation. The nurse assesses the client, completes the agreed service, documents findings, teaches the client or caregiver, and communicates authorized concerns.
6. Follow-up plan. The family receives practical next steps, including what to monitor, whom to call, and when another visit or a higher level of care may be needed.

Important Service Boundaries

• RN care management is not a substitute for a primary-care provider, specialist, pharmacy, home-health agency, emergency department, or 911.
• We do not diagnose medical conditions, prescribe medications, or independently change prescribed treatment.
• Hands-on skilled services are not automatically accepted. Availability depends on assessment, current orders when required, nurse competency, insurance coverage, staffing, supplies, and the safety of the setting.
• Physician communication requires the appropriate client authorization. We cannot guarantee how quickly an outside provider will respond.
• Private-pay services are not represented as Medicare-covered or Medicaid-covered services. Each payer, VA authorization, long-term-care policy, or managed-care contract must be confirmed separately.
• Routine scheduled visits are not emergency or 24/7 on-call coverage unless a separate written agreement specifically provides it.

Frequently Asked Questions

Can the nurse speak with my parent’s doctor?

Yes, after the client or authorized representative signs the necessary communication authorization. The nurse can organize observations and questions, provide updates, request clarification, and document new directions. The nurse does not prescribe or independently change the physician’s plan.

Do wound care, insulin, medications, or IV services require an order?

Many hands-on treatments require a current written order or clearly documented prescriber direction. Elder Empowerment verifies the treatment, medication, dose, route, frequency, parameters, supplies, and provider contact before accepting the visit.

Can you help us find assisted living or another care setting?

Yes. Placement navigation can include a care-needs assessment, option comparison, short-list development, tours or meetings, records coordination, transition support, and follow-up. We do not guarantee admission, bed availability, payer approval, or a specific result.

Can we purchase nurse oversight without using Elder Empowerment caregivers?

Yes. Private RN care management may be purchased as a stand-alone service. It can also be coordinated with Elder Empowerment personal care, respite, homemaking, and companion services when the family wants one organized plan.

Is private RN care management covered by insurance?

This page describes private-pay services. Some long-term-care insurance plans, VA authorizations, or other programs may reimburse eligible services, but coverage must be verified before service. Elder Empowerment does not guarantee reimbursement.
